With the Life Plan Module you can automate many of the steps in the case management process in your organization/program. The life plan system improves efficiencies by tracking a program participant's progress through the entire life plan.
Since the life plan is created through the integration with our survey system, an extensive database of all program participant specific assignments and their status is maintained; including sign off dates with a participant's youth leader and final approval by the participant's mentor.
Our Housing Link module manages all housing which is available for the program participants. Information such as address, amenities, type of housing, and rental fees can be entered into the system by property managers who are participating in the program. Program participants can then search for housing based upon parameters such as location, costs, amenities, etc. and receive a list of all housing that meets their criteria.
The Mentoring Module is a database of all personnel within the program which have been entered/identified as Mentors; a very critical role within the program. In addition to the basic personal information, demographic information such as geographical location, areas of expertise or business experience, etc. can be defined. Program participants have the ability to search the Mentor database to identify those mentors which can provide the specific assistance or service they need.
Provides the opportunity for program participants to post their own personal testimonials for other program members to read. To ensure the testimonials are appropriate, the actual testimonial will be submitted by the program administrator.
To facilitate the sharing of personal need requests among the program participants, a special Personal Need Request link has been established. To ensure the personal need requests are appropriate, the actual request will be submitted by the program administrator.
